Travel Licensed Practical Nurse (LPN)

Position Overview

We are seeking a compassionate and experienced Travel LPN to join a supportive long-term care and skilled nursing team in Tennessee. This is an excellent opportunity for an LPN who enjoys providing hands-on patient care while working with Short-Term Rehab and Long-Term Care residents. The primary assignment is within the Skilled Nursing population, with approximately 17 skilled beds and 55 long-term care beds. Travelers may be asked to float to Long-Term Care based on facility needs.

Key Responsibilities

• Provide safe, compassionate, and individualized nursing care
• Complete nursing rounds and ongoing patient assessments
• Administer medications and perform medication passes
• Provide direct patient care and monitor changes in condition
• Supervise and collaborate with CNAs
• Perform wound care as needed
• Support Short-Term Rehab and Long-Term Care residents
• Communicate effectively with the interdisciplinary care team
• Maintain accurate and timely documentation in PointClickCare
• Follow facility policies, procedures, and patient-safety standards

Requirements

• Active Tennessee LPN License
• BLS certification
• Minimum 1 year of Long-Term Care experience
• Nursing home experience required
• Strong medication administration/medication pass skills
• Strong patient-care skills
• IV Certification preferred
• Experience with Short-Term Rehab and Long-Term Care residents preferred
• Able to supervise CNAs effectively
• Comfortable working a 7:00 PM – 7:00 AM night shift
