Travel Licensed Practical Nurse Long Term Care Job

Responsibilities

• Administer prescribed medications and monitor for adverse reactions per physician orders
• Assess residents, document findings, and update care plans with the interdisciplinary team
• Provide routine nursing care including vital signs, wound care, and wound assessment as appropriate
• Assist residents with activities of daily living and maintain dignity and safety
• Collaborate with RNs, LPNs, CNAs, and physicians to coordinate individualized care
• Document care delivered accurately using electronic health records

Requirements

• Active LPN license to practice in Vermont (or eligible to practice in VT)
• Experience in long-term care or geriatric populations preferred
• Ability to work in a long-term care setting and handle a flexible assignment
• Strong communication and documentation skills
• Proficiency with electronic health records or charting systems
