Travel Licensed Practical Nurse Long Term Care Job

Travel assignment through Healthforce Healthcare LLC in Concord, New Hampshire, beginning 07/27/2026 for 13 weeks.

About the role

Provide nursing care in a long-term care (nursing home) setting. This is a temporary travel position with guaranteed 40 hours per week on an evenings shift (3:00 PM–11:30 PM), including every other weekend.

Responsibilities

• Provide direct, compassionate nursing care to residents following individualized care plans and facility policy.
• Administer prescribed medications and treatments within the scope of LPN practice; monitor responses and document outcomes.
• Perform routine assessments, monitor vital signs, wound care, and early recognition of clinical changes; escalate to the supervising RN or physician as appropriate.
• Maintain accurate and timely documentation in the resident’s medical record; communicate changes to the care team clearly and promptly.
• Collaborate with RNs, CNAs, and other members of the interdisciplinary team to support safety, comfort, and quality of life for residents.
• Assist residents with activities of daily living as needed, while promoting independence and dignity.

Requirements

• Active Licensed Practical Nurse (LPN) license in New Hampshire, or eligibility to practice per state requirements.
• Experience in a long-term care or skilled nursing setting preferred.
• Ability to work the evenings schedule (3:00 PM–11:30 PM) and fulfill the every-other-weekend requirement.
• Willingness to participate in a 13-week travel assignment with guaranteed hours.

Pay

Weekly pay ranges from $1,311 to $1,427.

Schedule

• Shift: Evenings, 3:00 PM–11:30 PM.
