Seeking a Licensed Clinical Social Worker (LCSW)
Responsibilities
- Provide discharge planning, psychosocial interventions, and social casework to patients and families.
- Assess patients' social, emotional, and financial needs, developing long- and short-term goals.
- Collaborate with healthcare teams to understand social and emotional factors affecting patient care.
- Document services in compliance with regulations and payer guidelines.
- Manage policies related to patient social care and advocate for patients' needs.
Qualifications & Skills
- LCSW credential required; independent decision-making skills essential.
- Excellent communication skills with staff, physicians, patients, and families.
- Preferably hold a Master’s degree from a CSWE-Accredited School.
- Minimum of 2 years of relevant work experience.
