About the role
A licensed clinical social worker responsible for providing comprehensive mental health and care coordination services.
Responsibilities
- Conduct initial evaluations
- Develop and revise individualized care plans
- Document patient progress
- Facilitate discharge planning
- Educate patients and caregivers
- Participate in care conferences
- Ensure documentation meets coverage criteria
Qualifications
- Active LCSW certification
- BLS certification
- At least one year of experience
- Skills in care coordination, collaborative care, patient education, and documentation
Pay & Benefits
- Weekly pay ranging from $2,007 to $2,237
- 13-week assignment
