Responsibilities
- Conduct psychosocial evaluations within 24 hours of admission
- Care coordination and psychosocial assessments in a fast-paced acute care hospital setting
- Collaborate with healthcare teams
- Arrange community resources
- Maintain documentation using Epic EMR
Qualifications
- Master's Degree in Social Work (MSW)
- At least 1 year of recent inpatient hospital experience
- Experience with Epic EMR
- Current BLS certification
- Prior travel assignment experience
