Health Requirements
- Required to have certain vaccinations and health screenings completed for onboarding, including MMR, Hepatitis B, Varicella, Tdap, and a 12 Panel Drug Screen.
Professional Requirements
- Provide a driver's license/State ID, Ascension Profile, and two professional references for submission.
Background Check
- Ascension Criminal Background Check Results and EPLS required during onboarding.
Employment Verification
- Ascension Employment Verification and Confirmation of Assignment required during onboarding.
Optional
- Influenza Vaccine, Positive PPD-Negative Chest X-Ray and Annual TB Questionnaire, and Hiring Manager Evaluation are optional for onboarding.
