Position summary
RN Case Manager – Hospice
Position duties
- Assesses patients, develops and coordinates individualized hospice care plans
- Manages symptoms and pain
- Provides nursing care and education
- Collaborates with the interdisciplinary team
- Supports patients and families to ensure comfort and quality of life during end-of-life care in the home setting
Minimum required qualifications
- VT/compact RN license
- BLS
- 1+ year home health hospice case management experience
- Oasis experience
Schedule
- Length of assignment: 16 weeks
- Shift / hours per week: Days, M-F with some weekend coverage, 36 hours per week
- Shift: 12 hours

Pay Rate: $1800.00 - $2500.00
