Description

Home health setting with patient visits conducted in residential environments
Caseload expectation of 6 points per day or 30 points per week
Day shift schedule, Monday–Friday, 8:00 AM–4:30 PM (5x8-hour shifts)
Patient population includes geriatric, orthopedic, neurological, and cardiopulmonary diagnoses such as total hip/knee replacement, post-stroke rehab, CHF, COPD, and dementia
Floating may be requested based on facility needs

Requirements

• Minimum 1 year of recent home health PT experience required
  
• Proficiency in OASIS documentation required
  
• Current BLS certification required (must be through the American Heart Association)
  
• Active PT license required if applicable
  
• Strong skills in gait training, therapeutic exercise, functional mobility, and patient assessment
  
• Experience with standard home health equipment including walkers, gait belts, crutches, and assistive devices
  
• SSN and a copy of a valid driver's license required for consideration
  
• One supervisory reference required for consideration
  
• Prior permanent employees must have been separated from the facility for at least 6 months before being considered

About the Role

Responsibilities include evaluation, plan of care development, OASIS completion, reassessments, and discharge documentation
Business casual attire or solid color scrubs are acceptable
Local candidates are welcome and accepted at the same rate
RTO should be kept to a minimum — no full calendar weeks off preferred, and no more than 7 days total during the assignment; all RTO must be submitted upfront and no additional RTO will be approved after an offer is made
Candidates may not simultaneously hold a travel assignment and a per diem contract with the same health system; one must be chosen

Benefits

Pay package is based on 8 hour shifts and 40 hours per week (subject to confirmation) with tax-free stipend amount to be determined
