Travel Home Health Physical Therapist - Full-Time

About the role

Seeking a travel Home Health Physical Therapist to provide patient care in a home health setting.

Responsibilities

• Conduct start-of-care assessments, recertifications, therapy visits, and occasional safety or teaching visits for hospice patients.
• Maintain documentation using Home Care Home Base EMR system.
• Support adult and pediatric patients with a typical caseload of 5‑6 visits daily, working Monday through Friday with no weekends or holidays.
• Perform therapy visits, patient assessments, documentation, and occasional evaluations, ensuring quality care in a home health environment.

Qualifications & Skills

• Active ND PT license or compact license.
• At least 1 year of experience.
• Home health and OASIS experience preferred.
• Current BLS certification.
• Valid driver’s license and auto insurance required.
• Ability to provide own transportation and live at least 50 miles from the facility.
• Excellent communication skills and ability to work independently.
