About the role

Provide patient care as a Home Health Physical Therapist in a travel capacity. Deliver exceptional therapy services, adhere to treatment plans, and ensure patient well-being.

Responsibilities

• Deliver high-quality physical therapy services to patients in their homes
• Follow and implement individualized treatment plans
• Assess patient progress and adjust care plans as needed
• Maintain accurate documentation of patient care and therapy sessions

Requirements

• Active and valid Physical Therapist license
• Experience in home health or travel therapy preferred
• Strong skills in patient assessment and care planning
• Ability to work independently and manage a caseload in a travel setting

Schedule

40 hours per week, typically on an 8-hour shift. Potential for contract extension based on performance and need.
