Pioneer Healthcare Services is seeking a travel Home Health Physical Therapist for a 13-week assignment in Santa Maria, California. This position offers 40 hours per week, 8-hour day shifts, in a home health setting.
Responsibilities
- Assess patients' physical abilities and develop personalized treatment plans in the home setting
- Provide therapy to improve mobility, strength, and functional independence
- Collaborate with patients, families, and caregivers to support rehabilitation goals
- Educate patients and families on exercise programs and safety measures
- Monitor and document patient progress and adjust treatment plans as needed
- Implement interventions tailored to the home environment and lifestyle
- Coordinate care with healthcare teams and community resources for comprehensive support
Requirements
- Doctorate in Physical Therapy
- Active California Physical Therapist (PT) license
- Current BLS/CPR Certification
- Clean driving record
Pay
$2,489.72 - $2,566.72 per week

- 13-week assignment
- Day shifts, 5 days per week, 8 hours per day (up to 40 hours per week)
