This opportunity allows you to provide individualized, one-on-one care that helps patients regain independence and improve their quality of life while working with one of the nation's leading home health providers.Job ResponsibilitiesComplete comprehensive physical therapy evaluations in patients' homes.Develop individualized treatment plans tailored to each patient's goals and functional needs.Treat patients recovering from orthopedic procedures, neurological conditions, cardiopulmonary diagnoses, and generalized weakness.Educate patients and caregivers on mobility, home exercise programs, fall prevention, and safety.Collaborate with physicians, nurses, occupational therapists, speech therapists, and case managers.Complete accurate and timely documentation using Homecare Homebase (HCHB).Travel throughout the assigned territory to provide exceptional patient care.QualificationsActive New Jersey Physical Therapist license (or ability to obtain prior to start)Minimum of 1 year of recent Home Health experienceOASIS documentation experience requiredHomecare Homebase (HCHB) experience strongly preferredCurrent BLS certificationValid driver's license, reliable transportation, and current auto insuranceStrong communication and organizational skillsAbout Amedisys Home HealthAmedisys is one of the nation's leading providers of home health, hospice, and high-acuity care.
