About the role
Ardor Health Solutions is seeking a Home Health Physical Therapist for a 13-week contract with Amedisys in Crestview Hills, Kentucky.
Responsibilities
- Evaluate and treat patients in their homes
- Develop individualized plans of care
- Complete reassessments
- Provide gait, balance, strengthening and mobility interventions
- Educate patients and caregivers
- Cook up with the interdisciplinary Home Health team
Requirements
- Graduate of an accredited Physical Therapy program
- Active Kentucky PT license or ability to obtain prior to start
- Current BLSPrevious Home Health experience is required
- Homecare Homebase experience preferred
- Comfortable traveling throughout the assigned territory
- Strong independent case-management and documentation skills
- Guaranteed weekly per diems—even during approved time off
