Travel Home Health Physical Therapist - $2,498 per week

About the role

The Physical Therapist will be responsible for assessing and providing therapy services to patients, developing individualized care plans, and ensuring optimal functional independence and safety. The role includes monitoring schedules, coordinating care, supervising staff, and maintaining documentation in compliance with regulations and standards.

Responsibilities

  • Actively monitor and amend patient visit schedules to maximize productivity.
  • Perform comprehensive assessments including psychosocial, educational, safety, and risk factors.
  • Develop and document plans of care with patient-specific goals and logical interventions.
  • Provide skilled physical therapy treatments and assess patient status and progress.
  • Carefully coordinate care with a multidisciplinary team for optimal patient outcomes.
  • Supervise clinical activities of PTAs, students, and home care aides.
  • Complete and coordinate required paperwork for compliance with regulations.
  • Participate in in-service training programs for customers and community groups.
  • Engage in marketing and promoting rehab services in the community.
  • Carry out various patient-related health assessments and procedures.
  • Maintain timely communication regarding changes in patient care schedules.
  • Ensure proper coordination of safe patient discharges or transfers.
  • Participate in peer review audits, providing and accepting feedback professionally.

Requirements

  • Current Physical Therapist licensure or eligibility.
  • Experience in home health, rehabilitation, or related settings preferred.
  • Strong knowledge of federal and state regulations and APTA guidelines.

Pay

Pay package is based on 8 hour shifts and 40.0 hours per week (subject to confirmation) with a tax-free stipend amount to be determined.

Schedule

Monday to Friday, 8 AM to 4 PM with 2-3 weekend days as needed.
