Travel Home Health Physical Therapist - $2,446 per week

Ardor Health Solutions is seeking a travel Home Health Physical Therapist for a 13-week contract in Franklinton, North Carolina.

Responsibilities

• Independently evaluate and treat Home Health patients
• Establish individualized plans of care
• Complete reassessments and Start of Care visits
• Provide skilled interventions focused on mobility, gait, balance, strengthening, transfers, safety, and functional independence
• Independently manage your daily caseload and territory
• Provide patient and caregiver education
• Cookice with the interdisciplinary Home Health team
• Complete timely HCHB and OASIS documentation

Requirements

• Graduate of an accredited Physical Therapy program
• Current North Carolina PT license or Compact privilege
• Current BLSPrevious Home Health experience required
• HCHB and OASIS experience strongly preferred
• Comfortable completing SOC visits
• Comfortable traveling throughout Vance and Warren Counties
• Strong independent case-management and documentation skills

• Previous Home Health experience required

Pay

Guaranteed weekly per diems—no time off
Double-time pay for hours worked between the facility guarantee and 40 hours

Schedule

M–F 8–5
No on-call
Approximately one Saturday/month for SOCs
