Details

Shift: Day shift schedule, 5x8 hours (8:00 AM – 5:00 PM)

Weekend coverage required 1–2 weekends per contract

Documents using BayCare Forms, BayCare Scheduling, and Ace Timesheet

Equipment provided includes a laptop, MiFi for internet access, and a portable printer

Requirements

• Minimum 1 year of physical therapy experience required; home health experience required
• Current BLS certification required
• Valid state PT license required (license required if applicable)
• OASIS training preferred
• Proficiency in assessment, documentation, ortho, neuro, and cardiopulmonary/circulatory care
• Competency in home exercise program development, patient/family education, medication management, safety and injury prevention, and billing
• Ability to recognize signs and symptoms of physical, sexual, and psychological abuse
• Valid driver's license and personal vehicle required to conduct home visits
• Covid vaccination required, or exemption form must be on file
• Flu shot required; candidates with a declination must wear a mask at all times while on assignment
