Travel Home Health Physical Therapist - $2,418 per week
Ardor Health Solutions is seeking a travel Home Health Physical Therapist for a 13-week assignment in Cascade Locks, Oregon. This role offers 40 hours per week with an 8-hour day shift.

This is a great opportunity for a Physical Therapist who enjoys independent patient care while surrounded by the incredible scenery of the Columbia River Gorge. The role involves providing one-on-one care in a home health setting with access to outdoor recreation and a rewarding, flexible work environment.
Responsibilities
- Perform comprehensive physical therapy evaluations in patients' homes.
- Develop individualized treatment plans based on physician orders and patient goals.
- Help patients improve mobility, strength, balance, safety, and functional independence.
- Provide education on home exercise programs, fall prevention, and safe mobility.
- Communicate patient progress and needs with physicians, caregivers, and the interdisciplinary team.
- Complete accurate and timely clinical documentation.
Requirements
- Active Oregon Physical Therapist license or Compact PT License.
- Home Health experience preferred.
- OASIS experience preferred.
- HCHB experience preferred.
- Current BLS certification.
- Valid driver's license and reliable transportation.
