About the role

We’re looking for a Physical Therapist to join a 13-week Home Health contract assignment in Denver, CO. This role offers excitement, fewer restrictions, and the opportunity to build meaningful relationships in the community while delivering one-on-one, high-touch care to patients in their homes.

Responsibilities

• Deliver one-on-one, high-touch care to patients in their homes
• Conduct skilled therapy visits and perform assessments
• Create and manage individualized care plans
• Collaborate with families and interdisciplinary providers to improve patient outcomes

Requirements

• 2 years of experience as a Physical Therapist
• At least 2 years of experience in the Home Health setting
• Valid Physical Therapy license
• Willingness to obtain Colorado licensure
• Local and traveler candidates welcome

Pay

Pay package is based on 8-hour shifts and 40 hours per week (subject to confirmation) with a tax-free stipend amount to be determined.
