Travel Home Health Physical Therapist - $2,391 per week

VyTalent Solutions is seeking a travel Home Health Physical Therapist for a contract position in Delco, North Carolina.

This is a travel contract role in a home health setting, designed to meet today’s workforce needs. The position offers an opportunity to make a meaningful impact in patient care.

Responsibilities

• Perform clinical assessments to identify patient needs and determine appropriateness of care.
• Document, prepare, review, and complete a Plan of Care (POC), progress notes, phone orders, and other clinical record documentation on an ongoing basis.
• Revise the POC based on evaluation and ongoing assessment data.
• Work in consultation with physicians and as part of an interdisciplinary team.
• Plan and provide appropriate treatment for neuromuscular, neurological, and orthopedic disorders based on medical referral and therapy assessments.
• Assist and instruct patients and families in transfer techniques appropriate to patient abilities.
• Instruct on the use of equipment such as walkers, wheelchairs, canes, braces, prosthetic devices, and appliances safely.
• Utilize appropriate therapy equipment specific to patient diagnosis and needs.
• Order and adapt rehabilitative equipment to meet patients' needs and home limitations.
• Provide patient and family education regarding therapeutic procedures based on age, condition, culture, and learning readiness.
• Perform and oversee other team members in patient care consistent with patient needs and desires.
• Apply infection control and universal precautions in patient care activities to prevent contamination and disease transmission.
• Accept clinical assignments consistent with education and competence.
• Perform other related duties as assigned or requested.

Requirements

• Current state license as a Physical Therapist.
• Proof of current CPR certification.
• Current driver’s license, reliable transportation, and insurance.

Pay

Pay range: $2,530.00 - $2,656.50 per week, based on 8-hour shifts and 40 hours per week. Tax-free stipend amount to be determined.

Schedule

40 hours per week, 8-hour shifts. Schedule details to be discussed with the recruiter.

Contract Details

Duration: 13 weeks. Start date: ASAP.
