Responsibilities

• Complete comprehensive home health evaluations and reassessments
• Develop individualized treatment plans to improve mobility and independence
• Treat patients with orthopedic, neurological, cardiac, and general medical conditions
• Educate patients and caregivers on home exercise programs and safety
• Collaborate with physicians, nursing staff, and interdisciplinary care teams
• Complete timely documentation using Netsmart EMR

Qualifications

• Minimum 1 Year Home Health Experience Required
• Netsmart EMR Experience Preferred
• Mileage Reimbursement: $0.35/mile
• Coverage Area: Tucson, Oro Valley, Marana, Sahuarita, Green Valley & surrounding communities Arizona
• AZ PT License Required
• BLS Required
