Aequor Allied is seeking a travel Home Health Physical Therapist for a position in Jackson, Tennessee.

Responsibilities

• Provide physical therapy services to patients in their home environment
• Develop and implement individualized treatment plans
• Evaluate patient progress and adjust treatment plans as necessary
• Document patient care activities and progress in accordance with regulatory requirements

Requirements

• Current licensure as a Physical Therapist in the state of Tennessee
• At least one year of experience in home health physical therapy
• Bachelor's degree in Physical Therapy from an accredited program
• Passing score on the National Physical Therapy Examination (NPTE)

Qualifications

• Excellent communication and interpersonal skills
• Ability to work independently and as part of a team
• Strong organizational and time management skills

Skills

• Proficient in using electronic medical records systems
• Familiarity with home health care protocols and standards

Pay

The pay package is based on 8 hour shifts and 40 hours per week (subject to confirmation) with a tax-free stipend amount to be determined.

Schedule

40 hours per week, 8 hours per shift, start date ASAP.
