About the Role
We’re looking for a Physical Therapist to join a 13-week Home Health contract. This role offers the opportunity to deliver high-touch, one-on-one care to patients in their homes while collaborating with families and interdisciplinary providers to improve patient outcomes.
Responsibilities
- Deliver skilled therapy visits and perform assessments in patients' homes
- Create and manage individualized care plans
- Collaborate with families and interdisciplinary providers to enhance patient outcomes
- Provide quality therapy services aligned with patient needs and safety standards

Pay
$2,200–$2,400 per week (based on 8-hour shifts and 40 hours per week, subject to confirmation).
