Job Description & Requirements
Supplemental Health Care is seeking a Home Health Physical Therapist for a 13-week contract starting ASAP in Franklinton, North Carolina. The position requires an active, unrestricted Physical Therapist license, current BLS certification, 1+ year of recent home health experience, and familiarity with Medicare-certified home health guidelines and OASIS-E documentation.
- Active, unrestricted Physical Therapist (PT) license in the state of assignment
- Current BLS certification (AHA)
- 1+ year of recent home health experience (within the last 2 years)
- Familiarity with Medicare-certified home health guidelines and OASIS-E documentation
- Experience conducting PT evaluations and developing therapy plans
- Experience with EMRs such as Homecare Homebase, NetSmart, or Epic
- Clear background check, driving record, and up-to-date health clearance (TB, immunizations)
- Reliable transportation with valid auto insurance and ability to travel between patient homes
Qualifications
Qualified candidates must have experience with gait and balance evaluations, home safety assessments, and mobility training. They should also be able to educate patients and caregivers on exercise programs, adaptive techniques, and safety strategies, and collaborate with RNs, OTs, SLPs, and physicians to ensure coordinated, interdisciplinary care.
Key Responsibilities
Home Health Physical Therapists are responsible for performing initial and ongoing PT assessments and developing individualized plans of care. They conduct skilled interventions for strengthening, endurance, pain management, and fall prevention, and provide education on exercise programs, adaptive techniques, and safety strategies. They document all care in compliance with Medicare, OASIS-E, and agency standards, and collaborate with interdisciplinary teams to ensure coordinated care.
