Travel Home Health Physical Therapist - $2,146 per week

About the role

The Physical Therapist will primarily focus on home health within the Physical Therapy department, with responsibilities including patient care and treatment planning. There is no clinical involvement required for this role, and an interview with the manager is part of the selection process.

Responsibilities

• Conduct start of care assessments, recertifications, and daily visits with an average of 6.5 points per day.
• Perform home health visits and manage typical workloads of 5-6 visits per day if the day does not involve travel or additional assessments/admissions.
• Attend to majority Home Health cases and occasionally conduct safety evaluations or educational visits in Hospice scenarios.

• BLS Certification
• Active ND Physical Therapy License
• Driver's License & Auto Insurance

Pay

The pay package is based on 8 hour shifts and 40.0 hours per week (subject to confirmation) with a tax-free stipend amount to be determined.
