Travel Home Health Physical Therapist - $2,138 per week

Responsibilities

• Assess patients' physical abilities and develop personalized treatment plans in the home setting
• Provide therapy to improve mobility, strength, and functional independence
• Collaborate with patients, families, and caregivers to support rehabilitation goals
• Educate patients and families on exercise programs and safety measures
• Monitor and document patient progress and adjust treatment plans as needed
• Implement interventions tailored to the home environment and lifestyle
• Cookordination care with healthcare teams and community resources for comprehensive support

Requirements

• Doctorate in Physical Therapy
• MI Physical Therapist PT license
• Current BLS/CPR Certification
• Clean Driving Record
