Travel Home Health Physical Therapist - $2,266 per week

Supplemental Health Care is seeking a travel Home Health Physical Therapist to provide compassionate, patient-centered care in Lewistown, Pennsylvania and the surrounding areas. About the role

This is a 13-week travel contract with a start date as soon as possible. The position requires 40 hours per week, with 8-hour shifts. Pay ranges from $2,080 to $2,266 per week, paid by the hour (not by the visit). The role involves 5–6 visits per day, each lasting 45–90 minutes based on complexity, with travel time factored into productivity expectations.

Responsibilities

• Perform initial and ongoing PT assessments and develop individualized plans of care
• Provide skilled interventions for strengthening, endurance, pain management, and fall prevention
• Conduct gait and balance evaluations, home safety assessments, and mobility training
• Educate patients and caregivers on exercise programs, adaptive techniques, and safety strategies
• Collaborate with RNs, OTs, SLPs, and physicians to ensure coordinated, interdisciplinary care
• Document all care in compliance with Medicare, OASIS-E, and agency standards

Requirements

• Active, unrestricted Physical Therapist (PT) license in the state of assignment
• Current BLS certification (AHA)
• 1+ year of recent home health experience (within the last 2 years)
• Experience conducting PT evaluations and developing therapy plans
• Familiarity with Medicare-certified home health guidelines and OASIS-E documentation
• Experience with EMRs such as Homecare Homebase, NetSmart, or Epic
• Clear background check, driving record, and up-to-date health clearance (TB, immunizations)
• Reliable transportation with valid auto insurance and ability to travel between patient homes
