About the role
Host Healthcare is seeking a travel Home Health Physical Therapist for a 13-week assignment starting on July 20, 2026, in Paducah, Kentucky.
Responsibilities
- Provide physical therapy services to patients in their homes
- Develop and implement individualized treatment plans
- Evaluate patient progress and adjust treatment plans accordingly
- Document patient progress and treatment sessions accurately
Requirements
- Current licensure to practice as a Physical Therapist in the state of Kentucky
- Experience in home health physical therapy preferred
- Excellent communication and interpersonal skills
- Ability to work independently and as part of a team
Qualifications
- Master's degree in Physical Therapy from an accredited program
- Passing scores on the National Physical Therapy Examination (NPTE)
- Current CPR certification

Pay
The pay package is based on 8-hour shifts and 40 hours per week (subject to confirmation), with a tax-free stipend amount to be determined.
