Aequor Allied is seeking a travel Home Health Physical Therapist for a position in Crestview Hills, Kentucky.

Responsibilities

• Provide physical therapy services to patients in their home environment.
• Develop and implement individualized treatment plans based on patient needs.
• Evaluate patient progress and adjust treatment plans as necessary.
• Document all patient interactions and treatment sessions accurately.
• Collaborate with other healthcare professionals to ensure comprehensive care.

Requirements

• Current licensure as a Physical Therapist in the state of Kentucky.
• At least one year of recent relevant work experience in Home Health Physical Therapy.
• Bachelor's degree in Physical Therapy from an accredited program.
• Current CPR certification.

Qualifications

• Strong clinical skills and knowledge of physical therapy techniques.
• Excellent communication and interpersonal skills.
• Ability to work independently and as part of a team.
• Passion for providing quality care in a home setting.

Skills

• Proficiency in using electronic health records (EHR).
• Experience with telehealth platforms.

Pay

The pay package is based on 8 hour shifts and 40 hours per week (subject to confirmation) with a tax-free stipend amount to be determined.

Schedule

Shifts are 8 hours long and will be scheduled based on the patient's availability.
