Responsibilities
- Assist in delivering evidence-based interventions such as ADL retraining, mobility, strengthening, cognitive activities, and adaptive equipment use.
- Follow established care plans, document patient progress, and collaborate with a multidisciplinary team including therapists, nursing staff, and social services.
- Support discharge planning and contribute to patient outcomes in a patient-centered environment focused on high-quality care.
Qualifications & Skills
- Certified as an Occupational Therapy Assistant with required license and SNF experience.
- PREFERRED: Travel experience.
- Ability to work in a team-oriented, supportive environment with good documentation skills.
