About the Role

Coordinate transportation and ensure the safe, timely transport of residents to and from physician appointments under the supervision of licensed nursing personnel.

Responsibilities

  • Organize transportation of residents to complete required appointments based on priority of resident needs.
  • Transport residents to and from inpatient and outpatient programs, appropriate facilities, and residences.
  • Develop driving schedules based on residents’ scheduled medical appointments and program destinations.
  • Provide transportation for current or new residents from hospitals after discharge.
  • Provide reports, information, and assessments of residents’ behavior or changes in condition during transportation upon return to the center.
  • Report changes or abnormal findings in resident status, such as consciousness, pain, bleeding, and vital signs that occur during transport.
  • Observe residents carefully and report changes in condition to the clinical supervisor.
  • Transport medical records for signatures to medical offices.
  • Maintain a high level of safety following safety policies and procedures while in the center and transporting residents.
  • Maintain the van in a neat, clean, and good working condition at all times; ensure routine maintenance is performed, log mileage, and keep the vehicle clean.
  • Assist clinical staff with safe lifting, transferring, and/or transporting residents as specified by physician orders and/or clinical supervisor.
  • Assist in the care of residents requiring moderate to total assistance in moving.
  • Establish positive communications with outside facilities, hospitals, physicians' offices, and specialty clinics to ensure proper services for residents are maintained and scheduled appropriately.
  • Promptly report any scheduling changes to the Director of Nursing (DON), nursing staff, ancillary service providers, and responsible parties, and take follow-up action as necessary.
  • Provide adequate hydration to residents at regular intervals each day according to specific resident needs.
  • Protect the personal property of the residents of the facility.
  • Attend and participate in scheduled training, educational classes, and meetings to maintain current certifications as applicable.
  • Adhere to work schedules in completing and performing assigned tasks; complete all assigned tasks in a timely manner as instructed by the direct supervisor.
  • Meet acceptable center attendance requirements; notify the facility before late arrival or absence from work according to established policy.
  • Cooperate with other facility personnel to ensure services are adequately maintained to meet the needs of residents.
  • Create and maintain an atmosphere of warmth, personal interest, and positive emphasis, as well as a calm environment.
  • Adhere to established employee policies and dress code.

Requirements

  • Certified in the state of practice.
  • Successful completion of a State Approved Nurse Aide Training and Competency Evaluation Program; name must appear on State Registry as a Nurse Aide in good standing.
  • Current BLS/CPR certification (required for all Skilled Nursing Facilities; Assisted Living Facilities as required by state).
