Responsibilities

• Maintains professional communication with internal and external contacts, including referring physicians and transferring hospitals.
• Evaluates hospital bed availability and develops contingency plans to ensure uninterrupted daily operations.
• Oversees patient placement operations to ensure timely admissions and transfers according to hospital policies and patient care requirements.
• Continuously reassesses patient placement requests and adjusts staffing to manage capacity demands across the system.
• Uses complex acuity algorithms to triage and process transfer requests, ensuring appropriate level of care and safe patient transport.
• Documents transfer processes accurately in software systems to meet regulatory compliance and improve patient safety.
• Captures psychiatric patient placement, ensuring legal documentation is accurate and appropriate.
• Provides education and training to new employees, participates in staff meetings, and stays current with operational processes.
• Supports Mission Control Departmental functions and participates in community service activities.
• Updates and maintains waitlists for patients awaiting placement in a receiving facility, tracks community daily census for placement opportunities.
• Ensures legal documentation is accurate and appropriate prior to transfer of patients.
• Verifies insurance benefits on all patients prior to admission and engages Emergency department and medical floor nursing team, Patient Access and Patient Financial Services in the verification process.
• Aids in training/mentoring new employees, attends staff meetings, and maintains current knowledge of operational processes.
• Reviews electronic communications to keep abreast of process and departmental changes.
• Performs other duties as assigned.

Requirements

• Proficient verbal and written communication skills
• Proficient in computer database skills
• Proficient in real-time documentation in electronic format
• Associate's degree in Nursing
• Bachelor's degree in Nursing (preferred)
• Registered Nurse (RN) license

Qualifications

• 2+ years of relevant acute care experience
• Behavioral Health Patient Placement experience (preferred)
• Transfer Center experience (preferred)

Skills

• Proficient verbal and written communication skills
• Proficient in computer database skills
• Proficient in real-time documentation in electronic format

Pay

$32.76 - $57.47
