Transplant Pharmacist - PRN

Join Sanford Transplant as a PRN Pharmacist and play a vital role in helping patients achieve life-changing transplant outcomes. You'll work alongside a highly specialized, collaborative team while maintaining the flexibility of a PRN schedule.

Responsibilities

• Review prescriptions to assure accuracy, determine needed ingredients, and evaluate their suitability.
• Provide information and advice regarding drug interactions, side effects, dosage, and proper medication storage.
• In a retail/community pharmacy setting, review, interpret, and dispense both prescription and non-prescription medication to patients.
• Respond to prescription issues and advise providers on the selection, dosage, interaction, and side effects of medications.
• Counsel patients on medication prescriptions, including proper administration, possible side effects, expected response, storage, and any other patient questions.
• Monitor the health and progress of patients in response to drug therapy.
• Learn and utilize new technology as the profession of pharmacy evolves.
• Provide oversight and clinical supervision for pharmacy services and pharmacy support personnel.
• In a medical center or institutional setting, review and interpret provider orders, provide drug information, assist in problem solving, and provide other aspects of pharmaceutical care.
• Perform duties such as unit dose dispensing, IV admixtures, outpatient prescriptions, navigating the electronic medical record, compounding, and working with automated dispensing devices.
• Utilize appropriate age-related patient information to assess pharmaceutical needs, including neonatal, pediatric, adolescent, adult, and geriatric patients.
• Follow all established regulations and standards governing the practice of pharmacy.
• Act as a shift supervisor under direct or indirect supervision by department managers as required.
• Demonstrate good written and verbal communication skills.
• Participate in pharmacy services across acute care surgery, neurology, cardiology, pediatrics, oncology, infusion services, intensive care, general medicine, dialysis, transplant, antimicrobial stewardship, and medication reconciliation.
• Perform decentralized clinical functions such as direct patient interaction, interdisciplinary team rounding, chart reviews, patient counseling, and collecting accurate medication lists.
• Perform centralized clinical functions such as checking and dispensing medications and overseeing the clinical functions of the IV room.
• In the transplant setting, manage immunosuppressive therapies, perform therapeutic drug monitoring, identify medication interactions, provide patient and caregiver education, and collaborate with the multidisciplinary transplant team to optimize graft function and patient outcomes.

Qualifications

• Graduate of an accredited School of Pharmacy.
• Thorough knowledge of drugs and their actions.
• Knowledge of state and local regulations.
• Hospital experience or residency preferred.
• License must be in good standing by the State Board of Pharmacy in state(s) of practice and/or possess multi-state licensure privileges as required by position.
