Transplant Coordinator

At Stony Brook Medicine, a TH CTS Senior Transplant Specialist / Transplant Coordinator is a valuable member of our team, providing clinical services to our patient population. Responsibilities

• Work with the pre-transplant multi-disciplinary team to conduct new patient evaluations and re-evaluations of patients on an annual basis.
• Coordinate patient entry and introduction to the transplant program.
• Coordinate pre-transplant education for patients and their caregivers.
• Work with the outreach coordinator to provide professional education for community nephrologists and dialysis center staff.
• Coordinate medical, surgical, financial, psychosocial, nutrition, and pharmaceutical evaluation and ongoing care needs of pre-transplant patients.
• Participate in patient care planning and selection conferences.
• List patients for transplant in a timely manner to facilitate access to donated organs.
• Help maintain the UNOS waiting list according to program policies and procedures, regulatory requirements, and national guidelines.
• Remain knowledgeable of all listed patients' status to ensure patient safety and facilitate organ placement in collaboration with transplant physicians.
• Work with the multidisciplinary team on performance improvement projects.
• Assess and manage potential living donors.
• Coordinate pre-living donation education for patients.
• Coordinate the medical, psychosocial, and financial evaluation of living donors.
• Attend national, regional, local, and internal meetings.

Requirements

• NYS licensed RN.
• Knowledge of Microsoft Office/Excel.
• Excellent communication skills.
• Critical care/ICU experience as an RN.

Qualifications (Preferred)

• CCTC (Certified Clinical Transplant Coordinator).
• Experience with ESRD/kidney transplant.

The role may be designated as "essential," requiring employees to remain at their work location or report to work during institutional emergencies to protect, recover, and continue operations.

Prior to the start date, the selected candidate must meet the following requirements:

• Successfully complete a pre-employment physical examination and obtain medical clearance from Stony Brook Medicine's Employee Health Services.
• Complete an electronic reference check with a minimum of three professional references.
• Successfully complete a 4-panel drug screen.
• Meet regulatory requirements for pre-employment screenings.
• Provide a copy of any required New York State license(s)/certificate(s).

Pay

The anticipated salary range for this position is $98,692 – $122,712 per year. The specific salary offer will be based on the candidate’s validated years of comparable experience. Some positions offer annual supplemental pay, such as location pay for UUP full-time positions ($4,000).
