Transitions of Care Pharmacist - Gulf Coast Medical Center
About the role
As a Transitions of Care Pharmacist at Lee Health, you'll serve as the key liaison between inpatient and outpatient pharmacy, ensuring patients have a safe, successful transition to post-acute care.
Responsibilities
- Work confidently across inpatient and outpatient electronic health records
- Review discharge medication orders and pend the discharge med rec
- Educate patients on medications and disease states prior to discharge
- Ensure patients have access to medications post-discharge
- Cover discharge prescriptions with outpatient pharmacy
- Coordinate with outpatient pharmacy on discharge prescriptions
- Review follow-up appointments and referrals for completeness
- Cover post-discharge needs with Care Management
- Follow up with patients after discharge to assess unmet needs
- Carry out core pharmacist duties preparing, dispensing, and monitoring medications across the facility
- Maintain a safe and effective medication-use environment for patients of all ages
- Provide direct supervision of technical support staff
Qualifications
- Doctor of Pharmacy degree required (BS Pharmacy accepted with equivalent, acceptable experience from an AACP-accredited College of Pharmacy)
- Completion of a pharmacy internship
- Active Florida Pharmacist License
Benefits
- Relocation assistance of up to $7,500 available for eligible pharmacists
Pay
Minimum to Midpoint Pay Rate: $51.54 - $67.01 / hour
Schedule
Shift: Shift 1/9:00:00 AM to 7:30:00 PM
