Job Summary
CoxHealth South is seeking an Ambulatory Care Pharmacy Specialist to provide pharmaceutical care services in assigned clinic or hospital areas. Responsibilities include designing, implementing, monitoring, evaluating, and modifying patient pharmacotherapies to ensure medications are indicated, effective, and safe. The position also involves participating in collaborative practice, protocol-driven practice, and supporting research activities.
Responsibilities
- Design, implement, monitor, evaluate, and modify patient pharmacotherapies
- Participate in collaborative practice and/or protocol-driven practice as appropriate
- Complete documentation for pharmacist activities
- Support the continuous improvement, patient safety, and accreditation programs of the department and institution
- Engage in additional responsibilities and teaching experiences
- Participate in special projects and committees
Requirements
- Graduation from an ACPE Accredited School of Pharmacy with a B.S. or Doctor of Pharmacy degree
- Preferred: Completion of a minimum of one year (PGY1) of pharmacy residency training or three years work experience in the outpatient setting
- Preferred: Completion of a PGY2 second-year specialty residency in Ambulatory Care
- Experience: Three years’ work experience in the outpatient setting if no pharmacy residency training
Qualifications
- Missouri Pharmacist license or eligibility for licensure within 90 days of hire
- MTM/MTS certification
