Transitions of Care Medical Assistant - Member Outreach

About the role
We want to help members navigate the healthcare system in a better and safer way. The Transitions of Care (TOC) Team supports members who are newly discharged from the hospital (and in some cases prior to discharge). You’ll serve as an advocate for these members, coordinating care and ensuring they have the necessary resources and support to achieve better health outcomes. As a TOC Associate your goal will be to support members in safely recovering at home and to prevent hospital readmissions. Our ideal TOC Associate is caring, compassionate, energetic, and genuinely enjoys connecting with people.
Responsibilities and Impact
- Calling (inbound/outbound) patients to check in after discharge to see how they are doing, and that they have received the assistance they need.
- Work with affiliated vendors to coordinate services and care for members, to include transportation, DME, meals, home health care and more as appropriate.
- Aid in scheduling post-discharge TOC APP, PCP, and Specialist visits.
- Collaborate with patients, providers, and caregivers to ensure a positive outcome.
- Work hand in hand with the other members of the TOC team to address patient needs towards team-based goals. This may include interdisciplinary huddles.
- Participate in and assist with pilot programs.
- Coordinate with medical providers and facilities to access necessary discharge records.
- Follow escalation processes to the clinical team to ensure patients get needed evaluation and care.
Requirements and Experience
- An active and clear Medical Assistant Certification
- 2-3 Years of Experience in healthcare, managed care, home health, and/or community health setting
Desired Skills and Experience
- Fully bilingual in English and Spanish
Pay and Benefits
The pay range listed for this position is the range the organization reasonably and in good faith expects to pay for this position at the time of the posting. The actual base salary offered will depend on a variety of factors, including the qualifications of the individual applicant for the position, years of relevant experience, specific and unique skills, level of education attained, certifications or other professional licenses held, and the location in which the applicant lives and/or from which they will be performing the job.
