Transitions of Care Ambulatory Pharmacist Supervisor
Job Summary
The University of Vermont Medical Center is seeking a Transitions of Care (TOC) Ambulatory Pharmacist Supervisor to oversee the Meds to Beds program and associated services in the Burlington, VT area.
Responsibilities
Develop and implement operational policies to address medication access barriers and liaise with relevant programs and resources to support patient transitions out of the hospital.
Implement the Meds to Beds program across various units within the medical center and collaborate with unit management to ensure its success and efficiency.
Oversee clinical triage and resolution of ambulatory prescription issues for both retail and Meds to Beds operations, and lead the continuous quality improvement program.
Work with the Manager, Outpatient Pharmacy to develop and implement additional wraparound services.
Requirements
Doctor of Pharmacy degree (or foreign equivalent) and at least one (1) year of pharmacist experience in a hospital-based ambulatory/Outpatient or transitions-of-care setting.
Experience coordinating multidisciplinary teams, including nursing, case management, hospitalists, and care transitions teams.
Experience with coordinating transitions-of-care services, such as medication reconciliation, discharge counseling, and high-risk medication review.
Experience developing staff training, competencies, or policy/procedure documents.
Experience managing insurance prior authorization and financial assistance workflows for discharge medications.
Ability and willingness to work days, evenings, weekends, and holidays, and be on call as needed.
VT pharmacist license.
