About the role
Supervise, support and train people with differing abilities served by Lumibility through group instruction. Implement activities that are person-centered and promote positive outcomes based on individual and group focused goals.
Responsibilities
- Ensure the health, safety and well-being of people served by Lumibility in accordance with the individual service plan and in compliance with agency policies and procedures.
- Provide supervision, guidance and instruction to individuals served by Lumibility at community-based assessment sites and during structured curriculum based instructional opportunities who are served under the Transitional Services program.
- Support services can include, but are not necessarily limited to: direct supervision, behavioral interventions, attending team meetings, skill development and providing effective communication with supported individuals at community assessment sites and businesses.
- Provide transportation for assigned service individuals in accordance with Lumibility safe-driving policies and practices.
- Maintain medical, social and programmatic files by recording each person’s progress and documenting other required information. This includes, but is not limited to: service goals, attendance records, behavioral data, progress notes, and other documentation as required or needed.
- Develop and maintain positive relationships with service participants, families, co-workers and other community representatives.
- Other duties, assignments and work locations as assigned
Requirements
- High school diploma or equivalent required; Bachelor degree in related field preferred; knowledge of learning and behavioral processes helpful.
- Must be over 18 and possess a valid Connecticut driver’s license, have an acceptable driving history and be able to utilize your own vehicle for Lumibility business as defined by Lumibility’s policy.

- Knowledge of person-centered, outcome-based services for people with differing abilities
- Knowledge of company policies and procedures and applicable regulatory standards
- Knowledge of community resources
- Skill in working with people with differing abilities
- Skill in operating standard office equipment, computer and various software packages.
- Skill in implementing programs & activities as outlined in the individual support plan
- Skill in communicating effectively in a courteous and professional manner
- Skill in writing clearly and concisely to maintain accurate records
- Ability to attend to the physical and personal needs of adults with differing abilities
- Ability to demonstrate good communication and interpersonal skills necessary to provide guidance and instruction to adults with significant challenges;
- Ability to work successfully, both independently and in a team environment
- Ability to problem-solve effectively
- Ability to maintain confidentiality
- Ability to use adaptive and other equipment
- Ability to maintain a patient and positive attitude
- Ability to drive and travel to varying locations as needed
Pay and Schedule
- Pay Rate: $19.50/hour
- Hrs: 40 hours/week

Qualifications
