About the role
We manage a caseload and implement Transitional Living services for youth in community scattered site housing. Essential duties include completing home and community visits, ensuring youth safety, implementing approved programming, coordinating services, assisting with transition plans, and being on call.
Responsibilities
- Ensure regular home and community visits are completed and documented per contract.
- Ensure safety of youth and environment per contract standards.
- Implement approved programming and curriculums to meet contracted standards.
- Cooktherapeutic and community services as needed.
- Aid youth in developing a transition plan portfolio including necessary adult documentation, resources, and skill acquisition.
- Complete required intake, assessment, and goal planning duties.
- Serve as after-hours, crisis On-Call as assigned.
Requirements
- Bachelor’s degree in human services field required.
- Must be over the age of 21.
- Experience in human services field is beneficial.
Qualifications
- Valid driver’s license with a risk-free driving record.
- Pass initial and periodic background screenings for child abuse/neglect, criminal history, sexual offenders, and child care/elder care.
- Submit fingerprints for state and FBI review.
- Pass a pre-employment drug screen and physical.
Skills
- Strong interpersonal and communication skills.
- Ability to work independently and as part of a team.
- Knowledge of human services principles and practices.
