Transitional Care Coordinator I or II, Full Time, Day Shift

Schedule: 8:00 AM – 4:30 PM, Monday through Friday

Requirements

• Bachelor of Social Work (BSW)
• Licensed Bachelor Social Worker (LBSW)
• CPR certification required within one month of employment

Qualifications

• Preferred: Master of Social Work (MSW) or Licensed Master Social Worker (LMSW)
• Preferred: One year of health care experience with discharge planning or care coordination
