Transitional Care Coordinator (CMA/RMA required)

This position is responsible for performing duties related to patient transitional care management (TCM). In-office presence is an essential function due to the need for real-time collaboration with providers, direct interaction with patients and caregivers, and secure handling of health data in compliance with HIPAA.
Qualifications
- Minimum completion from an accredited program for medical assistants (e.g., Certified Medical Assistant (CMA) or Registered Medical Assistant (RMA))
- Responsible for maintaining continuing education credits and renewal of certification prior to expiration
- CPR and First Aid Certified
- Passage of a criminal background check
- Understand and practice the ethics of confidentiality and requirements of the Health Insurance Portability and Accountability Act (HIPAA)
- Valid driver’s license and reliable transportation
Skills
- Computer skills using Microsoft (MS) Excel, SharePoint, MS Word, MS Outlook, MS Teams, Acumen, and various data management platforms used by partners of Eastern Nephrology Associates
- Ability to function well while involved in multiple task assignments
- Ability to understand and follow oral and written instructions
- Ability to meet set deadlines
- Willingness to accept orders and perform repetitive tasks
- Skill in organizing resources and establishing priorities
- Excellent verbal and written communication skills
Responsibilities
- Continuous monitoring of various informational sources (e.g., Health Information Exchange (HIE), hospital systems) to identify patient activity relating to discharge from an inpatient setting (30%)
- Provide interactive contact within two business days of discharge from an inpatient setting to perform required components of the TCM outreach (30%)
- Coordinate patient appointments for TCM face-to-face services, hospital follow-up, etc. (10%)
- Continuous monitoring of various informational sources to identify patient activity relating to recurrent emergency room use and repeat hospitalizations (10%)
- Provide patient care services in the clinic as assigned (10%)
- Other duties as assigned (10%)
Physical Demands
- Hearing, color vision, depth perception, field of vision, balancing, handling, and talking
- Extended periods in a stationary position
- Repetitive motion such as entering data into computer-based programs
- Position self with full range of body motion, manual dexterity, and eye-hand coordination
- Lift or move up to 10 pounds; use appropriate ergonomics and tools such as hand carts for heavier loads
Work Environment
Work is normally performed in a typical interior office environment. The noise level is usually moderate. Some office environments have stairs.
Schedule
Monday–Friday, 8:00 am to 5:00 pm with a one-hour lunch. Some after-hours work may be necessary to achieve the goals of the organization. Travel to other sites of Eastern Nephrology Associates may be required.
