Transition Facilitator (Youth Case Manager)

About the Role

GCBHS is seeking Case Managers/Youth Transition Facilitators to provide Community Psychiatric Support Treatment (CPST) services to severely and persistently mentally ill youth and young adults. CPST services are individualized, measurable, and designed to help individuals with mental illness and addictions lead healthy, productive lives. The position is ideal for recent Social Work, Psychology, Sociology, Criminal Justice, or related Human Services graduates, as well as MSW or Master’s in Mental Health Counseling students.

Responsibilities

• Provide case management – CPST services and Therapeutic Behavioral Services (TBS) to youths and young adults with severe mental illness in both outpatient and community settings.
• Initiate, coordinate, and monitor evaluations to identify client needs in areas such as living arrangements, finances, benefits, legal status, health, daily living activities, social supports, and recreation.
• Engage clients through assertive outreach, help them articulate their needs, offer services to meet those needs, and regularly reassess outcomes.
• Develop knowledge of mental health treatment resources, formal and informal agency linkages, client service utilization, associated costs, and alternative resource options.
• Understand the socioeconomic, cultural, gender, racial, and sexual orientation factors affecting service use, as well as the fiscal impact of service choices and the agency‑community resource system.

Requirements

• Masters degree in mental health, vocational training, education, social work, counseling, or BSW (Bachelor of Social Work), Bachelor in psychology, criminal justice, or a related human services field plus two years of specialized education, training, or experience in a relevant field.
• Experience in social services and behavioral health is preferred but not required.
• Frequent driving of personal vehicle required; a valid driver’s license, reliable vehicle, and insurable driving record are mandatory.

Pay

• Productivity bonuses up to $50 per hour.
