UPMC Home Health is hiring a Full-Time Physical Therapist for the Mon Valley and the surrounding areas. You'll work in a home setting with a diverse patient population, providing direct, one-on-one patient care. You'll collaborate with a team of specialized nurses, rehabilitation therapists, social workers, dieticians, aides, and trained administrative staff to deliver personalized care in patients' homes.

Responsibilities

• Oversee and support daily therapy operations, including patient visits, staffing coordination, referral oversight, and acting as a clinical resource
• Perform supervisory duties such as joint visits, documentation reviews, equipment oversight, orientation, and participation in meetings to ensure high-quality therapy services and operational efficiency
• Impact patient outcomes through appropriate visit and resource utilization, working closely with the Therapy Coordinator to guide and inspire therapy peers in optimizing care delivery
• Focus on customer service and continually strive to perform duties in a manner that results in optimal patient satisfaction
• Continually develop professional skills by assuming responsibility for ongoing continuing education
• Evaluate, plan, and treat patients in an effective Physical Therapy Program, including the use of modalities and assistive devices within the Standards of Physical Therapy Practice
• Identify, develop, and participate in process improvement opportunities within the Agency's Home Health or Early Intervention programs that will enhance the quality of services provided
• Assign and supervise the Certified Physical Therapy Assistant, if applicable
• Attend compliance training and adhere to the organization's standards of conduct, policies, and procedures
• Perform duties and job responsibilities in a fashion that coincides with the service management philosophy of UPMC toward patients, visitors, staff, peers, physicians, and other departments within the organization
• Maintain and complete all required documentation, orders, and Point of Care or written documentation promptly and accurately within Agency and regulatory standards
• Data syncs (transmits) information on the same day, if applicable
• Advise and consult with the family, inter-intradisciplinary team members, physicians, and other professionals, as appropriate, to ensure optimal patient care
• Provide physical therapy services and participate in the development of a physical therapy plan of treatment, including modalities, frequency, and treatment in collaboration with the referring physician for patients from birth to geriatrics to assist patients in obtaining maximum functional independence within the standards of physical therapy

Qualifications

• Graduate of a Physical Therapy curriculum accredited jointly by the Committee on Allied Health Education and Accreditation of the American Medical Association and the American Physical Therapy Association
• OR is eligible for the National Registration Examination of the American Physical Therapy Association
• OR two (2) years of appropriate experience as a physical therapist, and has achieved a satisfactory grade on a proficiency examination conducted, approved, or sponsored by the U.S. Public Health Service (except that such determinations of proficiency do not apply with respect to persons initially licensed by a State or seeking initial qualification as a physical therapist after December 31, 1977)
