About the Role
Physical Therapist in a skilled nursing facility setting, responsible for evaluating and treating patients, developing and implementing individualized treatment plans, monitoring progress, collaborating with the care team, and educating patients and caregivers on therapeutic exercises and home programs.
Responsibilities
- Evaluate and treat patients within a skilled nursing facility
- Develop and execute personalized therapy plans
- Track and document patient progress
- Coordinate care with interdisciplinary team members
- Educate patients and caregivers on exercises and home programs
Requirements
- Active Physical Therapist license in New Hampshire
- Graduate of an accredited Physical Therapy program
- Current CPR/BLS certification
- Experience in skilled nursing or long-term care environments
Preferred Skills
- Prior travel or contract therapy experience
- Familiarity with SNF documentation
- Understanding of Medicare compliance requirements
