Therapy - Physical Therapist

About the Role

Evaluate and treat patients in a skilled nursing facility to restore function and enhance mobility. Develop and implement individualized treatment plans tailored to patient needs and goals.

Responsibilities

• Document patient progress, treatment outcomes, and clinical notes adhering to facility and regulatory standards.
• Collaborate with interdisciplinary teams to coordinate comprehensive patient care.
• Educate patients and caregivers on therapeutic exercises and home programs for continued recovery.

Requirements

• Active Physical Therapist license in Oregon or ability to obtain one.
• Graduate of an accredited Physical Therapy program.
• Current BLS/CPR certification.
• Experience working in a skilled nursing or long-term care setting.

Preferred Skills

• Experience with MDS assessments and PDPM documentation.
• Familiarity with electronic medical records used in SNF environments.
