About the Role

The Speech Therapist provides outpatient rehabilitation services primarily in patients' homes or care facilities, focusing on the older adult population.

Responsibilities

• Evaluate and treat communication and swallowing disorders
• Develop individualized care plans
• Coordinate with interdisciplinary teams to ensure comprehensive patient care
• Supervise support personnel
• Manage documentation and order supplies
• Ensure safety standards are met
• Promote functional communication skills and facilitate device use
• Conduct reexaminations to adjust treatment as needed

Qualifications & Skills

• Valid Speech Language Pathologist license in applicable state(s) or proof of license eligibility
• Reliable transportation or a valid driver's license
• Strong clinical assessment, treatment planning, and documentation skills
• Ability to collaborate with healthcare teams and adapt programs per patient needs
• Knowledge of communication, cognitive, and swallowing disorder therapies
