About the role

The Speech Therapist provides outpatient rehabilitation services, primarily in-home, for older adults in various settings such as private residences, assisted living, or independent living facilities.

Responsibilities

• Evaluating and treating speech, language, swallowing, and communication disorders
• Developing personalized care plans
• Collaborating with interdisciplinary teams
• Supervising support personnel
• Ensuring timely documentation
• Maintaining safety standards
• Facilitating the use of adaptive communication devices
• Assessing hearing deficits
• Adjusting treatment plans based on patient progress
• Perform patient assessments, develop and implement treatment plans
• Address communication, cognitive, and swallowing disorders
• Coordinate care with team members and referring physicians
• Supervise support staff and support clinical education for new grads
• Order supplies, maintain equipment, and ensure safety standards
• Document clinical activities and patient progress accurately
• Provide patient and caregiver education on therapeutic techniques and device use

Qualifications & Skills

• Valid Speech Language Pathologist license or license eligibility in practice state(s)
• Reliable transportation or valid driver’s license
