Schedule

  • Tuesday–Friday, 8:00 AM–6:30 PM
  • Must be available to work two evenings per week

Responsibilities

  • Provide case management and counseling services to adults and children/adolescents diagnosed with persistent mental illness in individual, couples, and family treatment settings
  • Deliver services both onsite and in the community/client home
  • Collaborate with a team of internal and external service providers to meet all client needs
  • Complete clinical assessments that meet program funder and administrative rule requirements
  • Refer clients for community-based case management services as needed
  • Provide case management services as medically necessary
  • Complete required documentation (IM+CANS, encounters, consents) on a timely basis
  • Meet minimum service provision and billing standards
  • Acquire and maintain required certifications and trainings
  • Adhere to AID’s Code of Ethics, policies, and procedures
  • Maintain professional judgment and performance regardless of personal issues
  • Promote awareness and respect for the diversity of clients, employees, families, and stakeholders

Requirements

  • Master’s degree in a human services field
  • License preferred (LPC or LSW)
  • At least one year of experience in behavioral health preferred
  • Ability to navigate stairs on a regular basis
  • Tolerance of unkempt home conditions of clients
  • Working knowledge of Microsoft Windows applications
  • Flexibility to work evening hours
  • Use of personal automobile
  • Maintain automobile liability insurance as defined by AID’s personnel policies
  • Maintain valid Illinois driver’s license
  • Meet and maintain driving record standards as defined by AID’s insurance carrier
