Role Overview This position involves evaluating and assessing patients to establish functional, measurable goals aimed at restoring abilities. Provides treatment based on a written plan of care, monitors progress, and adjusts interventions as needed. Conducts discharge planning, including patient and family education, follow-up, and referrals to community resources. Documents all interventions and contributes to quality improvement initiatives. May supervise clinical students and support staff, fostering a caring environment that promotes health and healing. Participates in hospital committees and serves as a resource for performance enhancement. Key Responsibilities Assess patients using subjective and objective data.Develop and implement treatment plans aligned with patient goals.Evaluate treatment effectiveness and re-assess as needed.Coordinate discharge planning and patient education.Document care activities thoroughly.Support staff and student development.Engage in activities to improve healthcare delivery. Qualifications & Skills Minimum of 3 years of clinical experience or advanced training and certification.Basic computer literacy skills.Licensure including BLS certification within 14 days.Preferred certifications or advanced training in primary practice area.
